Notice:  Due to recent budget cuts, the Oklahoma Tax Commission is unable to mail forms to individuals or businesses.  Please visit our 2009 Income Tax Forms site to download and print any forms needed.

If you filed a paper resident income tax (511) form for Tax Year 2008, you will be mailed a 2009 packet in early January.  Otherwise, you will need to either download form(s) from our website or visit your area library.

Oklahoma libraries are being sent a CD of Oklahoma income tax forms that can be used to print the form(s) needed.
